Have you ever wondered whether your choice of underwear affects your sex life? Not your mood, but your physiology? A new study involving nearly 27,000 men seeking fertility assistance found that those who wear tight underwear more often have problems with sperm quality and potency. But let's sort this out without panic.

What was actually studied

Researchers surveyed 26,836 men who came for fertility assessment. Of these, 21,221 wore loose underwear, and 5,615 wore tight. Participants self-reported their preferred underwear type. Then their sperm quality was assessed by several parameters, and they were checked for erectile dysfunction and premature ejaculation using standard questionnaires. This is not an experiment but a cross-sectional snapshot: scientists simply compared the groups.

Tight underwear and sperm: what was found

After adjusting for various factors (smoking, alcohol, varicocele, prostatitis), it turned out that wearing tight underwear was associated with a number of impairments. Such men more often had lower ejaculate volume, reduced sperm motility (both total and progressive), a lower total number of motile sperm, and a smaller proportion of normally shaped cells. Additionally, they had a noticeably higher sperm DNA fragmentation index — a measure of genetic material damage. In plain terms: the difference is not catastrophic but consistent. For example, the risk of reduced motility was about 16% higher, and the risk of increased DNA fragmentation was about twice as high as in fans of loose underwear.

What about erection and ejaculation?

Differences were found here too. Erectile dysfunction occurred in 62.7% of men in tight underwear versus 57.8% in loose. Premature ejaculation — in 30.8% versus 23.2%. After adjusting for other factors, the association remained statistically significant, though not huge. This means that tight underwear may be one of many factors, but not the sole cause.

Why this might work

The main hypothesis: tight underwear raises scrotal temperature. Normal sperm production requires a temperature slightly below body temperature, which is why nature placed the testicles outside. Constant tight fitting may disrupt this temperature regime. Additionally, tight underwear may compress blood vessels and nerves, theoretically affecting erection. But let's emphasize: the study shows only an association, not a causal relationship. It is quite possible that men who choose tight underwear differ in other parameters that were not accounted for. Although the authors adjusted for smoking, alcohol, and some diseases, there remains a possibility that the issue is not only about underwear.

What this means for you

If you are planning a child or simply monitoring your sexual health, it might be worth reconsidering your wardrobe. Loose underwear is a simple and harmless way to slightly reduce risks. This is especially relevant for those who have already encountered fertility or potency problems. But don't rush to blame briefs for everything: sperm quality and erection depend on many factors — lifestyle, hormones, heredity, stress. If you have difficulties conceiving or in bed, it's better to discuss this with a doctor rather than limit yourself to changing underwear. But as an additional measure — why not?
